MANAGEMENT PROGRAM
Ideal for the top 10% of your health care utilizers, the Chronic Care Management Program is designed to assume the responsibility of a member’s primary care doctor. Through this high-touch care model, Alegis Care physicians directly provide and manage members’ care through multiple stages – from chronic care management to palliative care to transitional care.
COMPLEX CARE PROGRAM
Ideal for the top 15% of your health care utilizers, the Complex Care Program is designed to provide supplemental support for the members’ primary care doctor. Through this collaborative care model, Alegis Care nurse practitioners follow the care directives of network primary care doctors and provide recommendations for future care based on in-home services and observations.
HEALTH RISK ASSESSMENT PROGRAM
Alegis Care staff schedules in-home appointments to conduct comprehensive assessments for Medicare Advantage and Medicaid patients. These services may include medical history and examinations, specialized tests or screenings for specific disease states, or various services to close Gaps in Care.
