About

Jeffrey W. Fruin, DDS, DDS

Our goal at Dental Sleep Institute of Carlsbad is to educate patients about the grave risks associated with OSA and customize treatment that allows them to have normal, healthy sleep patterns.

We are excited to offer a new strategy to help our family of patients address the serious consequences of undiagnosed sleep apnea. We have purchased the take home sleep test device (Embletta) that gives a primary screening for sleep apnea. If we have patients whose answers to the INITIAL SLEEP SCREENING FORM indicate any probability of sleep apnea we will administer the test. If your results do not indicate sleep apnea the test is absolutely free of charge.

Sleep Apnea is defined as near 100% loss of oxygen flow for at least 10 seconds during sleep. There are two types of Apnea. The first, Central Sleep Apnea, is the least common and occurs because the central nervous system forgets to tell your body to breath. There is no gasping or choking, but the episode does interrupt the normal sleep cycle. The second and most common apnea is Obstructive Sleep Apnea (OSA). In OSA the upper airway collapses and blocks the flow of air to the lungs. These episodes can happen hundreds of times in a night and last for more than a minute. The magnitude of these incidents determines the severity of a patients OSA. When the flow of air is blocked, the blood oxygen levels drop and patients choke or gasp for air. Even though the patient may not even be aware of it, they are startled awake hundreds of times a night and never get the necessary sleep needed to function normally during the day. OSA is literally a psychological, emotional, and physical killer. The scientific evidence is staggering.

Oral appliance therapy (OAT) is a very effective means of treating mild to moderate cases of OSA. This therapy uses a mouth piece which positions the jaw slightly forward and prevents the airway from collapsing. Although all patients are not candidates for this therapy, its compliance rate is dramatically greater than the cumbersome CPAP. We have successfully treated many patients who were not compliant with their CPAPs using oral appliances.
A third treatment alternative is surgery. The types of surgery range from the removal of soft tissues including the uvula, to sectioning of the jaw and maxilla and moving them forward. Sometimes implants are placed in the back of the throat to hold the soft palate open. Although sometimes effective, these treatments are generally irreversible, painful, and generally have low success rates.

We consider it our primary objective to educate patients about their options and help them make the right choice in treatment. This requires a cooperative effort between dentist, sleep specialist, primary physician and possible ENT specialist. We believe that an inexpensive take home test to diagnose sleep related disturbances can be invaluable in strategizing treatment as well as determining the best way to utilize your insurance coverage.
