WARNING!!! DO NOT PLACE YOUR LOVED ONE IN THIS FACILITY!!!!!
My Mother who has severe dimentia and alzheimers and non communicative has been a long term resident at Lake Forest Nursing center since 2012. Since that time, I have had to repeatedly ask them to make sure that her face is cleaned, her outer ears are cleaned, her hands cleaned, to braid her hair etc. That's just the little stuff. NOW TO THE BIG STUFF. I popped in for a visit while she was in activities to find her leaning over in her wheel chair to the point that her left arm which was dangling beside the chair was turning bluish purple. I had to bring their attention to this after I gently lifted her arm back up and got her straightened up in her chair. Their response was a nonchalant, Oh! I'm sorry. A few months later, I was visiting her for lunch while she was being fed her pureed food. The gal feeding her seemed to be doing just fine, until she had to take care of someone else and instructed another gal to fill in for her. This new gal had no concept of the appropriate amount to feed a person that has limited control over swallowing. Here comes a HEEPING spoon full Pureed food for my mom to put into her mouth. I thought that she would just let a little go in but NO she pushed that heeping spoonfull in her mouth and expected my mom to manage it. Well that's when I sat there astonished at the fact that this aid just did this, and watched my moms face change from normal color to a little redder, a little more redder to purple thinking that this gal would help her! I jumped up and leaned my mom forward and started hitting her back to dislodge the puree until she finally coughed. I looked at the gal and asked her, "didn't you see she was choking on her food??" She just looked at me. I told her that if she sees someone face turning purple that usually means that they are choking. She said, I know". WELL WEREN'T YOU GOING TO DO SOMETHING OR JUST LET HER CHOKE TO DEATH!!!!!!!. Needless to say, I called the Ombudsman to come out and have a conference with the management of the facility and was under the impression that things would get better. NOPE! Just this past Saturday, a few months later, I get a phone call from the facility that my mom had a "little" accident and that there was "some" blood. Let me tell you, when I got to the ER and saw what had happened I wanted to scream! My wonderful Mother, My best friend had a 2 1/2" gash across her forehead deep enough that I could see her skull. Now you tell me, "little" accident??? NO, "some" blood. HELL NO! I felt helpless for my mother. Please do your research on these facilities, if you really care. I thought I had done mine but it is much harder to get the facts about a facility than you think.

If you need a place to rehab or to select one for a someone else, this is as good as they get. Staff are very attentive. My father, who is currently a resident, is clean and well cared for. The rehab is excellent and intensive, my dad has made huge strides in recovery and is confident that he will be recovered to the point of being able to go home and be independent. The staff is excellent and attentive; and most importantly very skilled. As DPOA, I don't encounter the usual difficulties in communication that seem to occur with getting information about an inpatient; they are aware of who I am and how I can help them. This is the third place my dad has been in and is by far the best.
